- Internship Approval Form
- Erasmus Student Work Placement Form
- Insurance Document (Accident and Liability Insurance)
- Halk Bank Euro Account Information (Copy of Passbook or Receipt)
- Transcript
- English Language Certificate (From the Foreign Languages Department)
- Visa (If applicable)
b) Documents to Be Submitted to the Erasmus Office After Mobility
- Certificate of Participation / Certificate of Attendance (An official document including the student's full name, internship dates, stating that the internship was within the scope of Erasmus Traineeship Mobility, and confirming successful completion of the internship by the host institution with its official stamp and authorized signature.)
- Internship Certificate & Evaluation Form
- Learning Agreement for Traineeship / After Mobility
- Copy of Passport:
Including pages with entry and exit stamps. - Participant Report: Report obtained from the Mobility Tool / Beneficiary Module.
